Service Description
- If you need the registered owner's name and address information for certain DMV transactions or other criteria listed on the form you will need to schedule this appointment and bring the following: *Government issued ID or Drivers License *Completed Request For Vehicle Record - To expedite the transaction please bring all requested documents and if possible bring the paperwork already filled out. This is a single transaction appointment, if you have other transactions that need to be processed please schedule the appropriate appointments separately.
Cancellation Policy
To cancel or reschedule, you must do so at least 24 hours prior to your scheduled appoint time to avoid a potential rescheduling fee of $20.00. Email appointments@susitnadmv.com with your appointment date/time and request to cancel or call 907-733-2266 and leave your appointment date/time in a voice message.
