If you do not have an original Alsaka Drivers License, Permit or Identification Card please bring the following to your appointment: • Origical Certified Birth Certificate or Passport • Any name change docuements • Proof of Physical Address • Knowledge Test (for driver/permit only) • Application for Alaska Drivers License, Permit or Identification Card (Click on Pencil to the right to access forms -->)
Cancellation Policy
To cancel or reschedule, you must do so at least 24 hours prior to your scheduled appoint time to avoid a potential rescheduling fee of $20.00. Email appointments@susitnadmv.com with your appointment date/time and request to cancel or call 907-733-2266 and leave your appointment date/time in a voice message.
